HOW TO REGISTER FOR THE SYMPOSIUM
You may register for the Symposium in one of the three following ways:

  • Express On-line Registration (see REGISTRATION ON LINE button on the main menu)
    The last day to register on-line is September 19, 2007.
    After this date participants will have to register on-site in New York.
    Additional registration forms for postal mail or fax use can be downloaded at this site.

  • Standard Registration via Postal Mail (Bank Check)
    Mail the completed Registration Form (see REGISTRATION FORM button on the main menu) with your payment to:

    Make check payable in US dollars drawn on a US Bank or International Money

  • Standard Registration via Telefax
    Fax the completed Registration Form (see REGISTRATION FORM button on the main menu) with your credit card information to:

METHOD OF PAYMENT (Registration Fee)
Payment must accompany submitted registration in order to be processed. The registration fees may be paid in one of the following ways:

  • Credit Cards (Visa, Master/Euro Card, American Express, and Discover).

  • Bank Checks must be payable in US dollars and drawn on a US Bank. Bank checks and International Money Orders should be made payable to DALM2007.  Purchase orders and telephone orders are not accepted.

COMPANY PAYMENTS
If a company pays on your behalf, please ask the company to indicate your name, your Email address and your full postal address (not the corporation’s address) on the registration form. If complete contact information is not provided prior to the Symposium, you will be asked to provide this information on-site before receiving your registration materials.

REGISTRATION CONFIRMATIONS
If you register on-line by September 19, 2007, you will receive an immediate on-line acknowledgement of successful submission of your registration with your Registration Number to be used for further contacts or to revise your data (please do not forget to print it for future reference). You will also receive a registration confirmation via email.

If a credit card transaction is unsuccessful, the DALM2007 Registration Office will promptly notify you.

If you register via postal mail or fax and the registration form is received by September 19, 2007, you will receive a Registration Confirmation Email with your Registration Number within a week to ten days. If an email address is not provided on the registration form, North American participants will receive their confirmation by postal mail; international (non-North American) confirmations will be sent via fax. Airmail will be used if fax or email is not possible.
In order to quickly resolve any problem when arriving at the Symposium’s Registration Desk, participants are kindly requested to bring with them complete documentation of the registration fee payment.

REGISTRATION DEADLINES

  • Registrations received by June 11 qualify for the reduced early registration fees.
  • Registrations at the regular registration fees must be received by September 19.
  • Registrations received after September 19 will be charged the highest on-site registration fees, and on-line acknowledgements and registration confirmations will not be issued prior to the Symposium.
  • On-site registration is also available during Registration hours on the 2nd floor promenade at the Hilton New York.

Registered Participants, Post-Doctoral Fellows, Students, and Nurses are entitled to:

  • Attend all Scientific Sessions
  • Attend the Welcome Reception
  • Attend Poster Viewing and Discussions
  • Attend the Scientific Exhibits
  • Continental Breakfasts and Coffee Breaks
  • Attend the Young Investigator Poster Awards and Poster Session Reception
  • Attend Optional Events (advance registration required)
  • Receive Final Program and Abstract Book

Admission to the Symposium Events is reserved only for participants wearing the Symposium badge

OPTIONAL EVENTS (included in all registration fees)
Limited seating available on a first-come, first-served basis; advance registration for event attendance required.

  • Friday, October 5 (7:00 - 8:30 a.m.) Breakfast Session: “Beyond Statin Monotherapy: Raising HDL-C, Lowering CVD Risk”
  • Friday, October 5 (12:30 - 1:30 p.m.) Lunch Session: “In Memoriam of Prof. David Kritchevsky” and “XXXIII Lorenzini Lecture”
  • Saturday, October 6 (12:45 - 1:45 p.m.) Lunch Debate: “Where Does Insulin Resistance Start?”
  • Saturday, October 6 (6:30 - 8:00 p.m.) Young Investigator Poster Awards presentation and reception.

BREAKFASTS AND COFFEE BREAKS
Continental Breakfasts and Coffee Breaks will be offered to all registered participants in the exhibition and poster areas and in the West Promenade area on the 3rd floor of the Hotel on Thursday, October 4 through Sunday, October 7.

REGISTRATION CHANGES
If you have already registered either on-line, by fax, or by mail, please send your requested changes by fax to (+1) 407-366-4138, or use the link to update a previous registration. Your Registration Number be included with your request.

CANCELLATION POLICY
Written notice of cancellation must be received by the DALM2007 Registration Office c/o Registration Systems Lab (see below) no later than August 31, 2007. If cancellation notice is received by this date, the registration fee will be refunded, less 30% for administrative charges. Cancellations received after August 31, 2007 will not be refunded; however, these registrants will receive their Symposium material by mail after the meeting. All approved refunds will be processed and issued 60 days after the Symposium.

PRIVACY LAW (For Italian participants)
Personal data will be handled exclusively by Fondazione Giovanni Lorenzini Medical Science Foundation according to the Italian Law 675/96 and law decree 196/2003.

PRESS REGISTRATION
Working press from newspapers, magazines, radio, television, and medical publications will receive a complimentary registration, provided that they submit a copy of their press credentials along with the registration form. Press registration may only be submitted by fax or postal mail.
